Bring the actual quote to the plan

What will your plan pay for?

Start with the exact visit, medicine and tests being proposed. Ask your insurer about each item and the medical reason for it. Keep its answer beside the quote.

Get ready · a learning step

Check coverage for the care on offer

Approval of a drug for a particular use does not guarantee payment by your plan, as of September 2026.

Note A

Ask whether the clinician and service are covered. Then ask separately about the exact drug, pharmacy and any tests.

If the insurer asks for prior authorization, it means a check before it agrees to pay.

Ask who submits that request, what records it needs and what you would owe if it is denied.

Put useful questions to the insurer

  • Does the answer cover this exact visit or only the medicine?
  • Is the proposed use included in the answer?
  • Does the answer cover the named pharmacy?
  • Which amount would I have to pay myself?
  • Can I have the reply and its reference in writing?

Ask for words you understand. If the quote changes, put the changed item back to the plan before relying on the earlier reply.

Using a health spending account?

Money for eligible medical expenses can be set aside in accounts called health savings accounts or flexible spending accounts.

As of September 2026, an expense for general wellness alone did not qualify as medical care under IRS rules.

Note B

The federal tax agency uses the short name IRS; its full name is Internal Revenue Service.

Ask your account administrator about the named expense and what proof it needs. Keep that question separate from whether an insurer covers the bill.

  1. Medicare example for weight-loss medicines

The Medicare source discusses medicines for weight loss. It does not say what Medicare will pay for your injury care. Ask your plan about the exact visit, drug and tests.
